Comparison of national and subnational guidelines for hand hygiene

手指衛生の普及は医療関連感染予防の礎石であると考えられる。この数年間、国際レベル、国家レベル、国内レベルの団体がそれぞれに手指衛生ガイドラインを策定している。これらのガイドラインを比較することにより、世界の諸地域の勧告およびその策定方法の理解が促進される可能性がある。検索エンジン、電子図書館、および個人的連絡によってガイドラインを特定し、欧州委員会第12総局の支援によるHARMONYプロジェクトのツールの修正版を使用して、これらのガイドラインの内容を分析した。検索された22のガイドラインのうち21について評価を行った。ガイドラインは、その領域、アプローチ、内容、および用語について様々であった。一部では忠告的な指示を主旨としており、その他では手指衛生実施の理由、タイミング、方法というような技術上の問題点に焦点を当てていた。エビデンスの収集・評価の程度には大きなばらつきがあり、詳細に記述されたものはごく少数であった。エビデンスと勧告の強度を示すグレード化システムと定義も様々であった。ガイドラインの目的は、医療における手指衛生実践を改善することにより、医療関連感染や抗菌薬耐性の減少を図ることであった。手指衛生の適用と手順に関する見解は概ね一致していたが、最善の実践法に関する勧告の範囲と水準は多様であった。ほとんどのガイドラインでは、遵守の評価法やガイドラインの有効性の評価のための監査法などの不可欠な要素が軽視されていた。結論として、エビデンスの徹底的な評価に基づく、世界のあらゆる地域で適用可能な勧告を作成するためにはより一貫したアプローチが必要である。勧告の実施および影響のモニターにかかわる要素について、より注視していく必要がある。

感染対策は手指衛生に始まり手指衛生に終わる、といい続けてはいるものの、ガイドラインだけでも様々であることから、要するにすべての医療従事者が手指衛生を完璧に実践するようになる決定打はないということが明らかになったと考えてよい。
