Epidemiology, medical outcomes and costs of catheter-related bloodstream infections in intensive care units of four European countries: literature- and registry-based estimates

欧州ではカテーテル関連血流感染（CRBSI）の発生率が高いにもかかわらず、疾患による負担についての情報は少ない。欧州4か国（フランス、ドイツ、イタリア、英国）を対象として、集中治療室（ICU）入室中のCRBSIによる医学的転帰および費用を推定するため、利用可能なデータのレビューを行った。これらのデータに基づいて、CRBSIに関連する上記4か国の死亡率と年間費用を推定した。その結果には各国間で大きなばらつきがみられ、CRBSI発生率は1.23～4.2件/1,000カテーテル日、8,400～14,400件/年、死亡は1,000～1,584例/年、CRBSIによるICU在室日数は15,960～201,600日、関連費用は3590万～1億6390万ユーロであった。これらのばらつきは、主として疫学調査の質の不均一性、および各国の臨床診療の多様性により説明される。

監訳者コメント：
欧州の4か国のCRBSIの現状を文献検索により調査した興味深い論文である。各国の医療体制の差によるところが大きい、在室日数や関連費用にばらつきがでるのは当然であるが、それらによる差がでにくい、純粋な感染制御のレベルを示すと考えられる指標であるCRBSI発生率が約3.5倍異なっていた。フランスが最も低く（1,000カテーテル日あたり1.2）、英国が最も高かった（同4.2）。英国のCRBSIの感染制御には大きな問題があるのかもしれないが、一方で確固たるサーベイランスによるCRBSIの確実な検知によるかもしれない。いずれにしても、こういったデータがないとBSI防止に関する議論が進まないので、貴重な情報といえる。
