Analysis of neonatal nosocomial infection rates across the Australian and New Zealand Neonatal Network

本稿では、オーストラリア・ニュージーランド新生児ネットワーク（Australian and New Zealand Neonatal Network）に参加している26施設の新生児院内感染症罹患率の相違について報告する。2002年から2004年に収集した出生時体重1,000 g未満の新生児計3,180例、260,694病院日のデータを解析した。出生時体重1,000 g未満の新生児の全感染症罹患率は、総入院日数1,000日あたり5.02［95％信頼区間（CI）4.75～5.30］で、既報データと同等であった。入院から35日間の打ち切りデータでは、感染症罹患率は1,000日あたり13.88（95％CI 13.14～14.65）であった。次に、各施設の標準化推定感染症罹患率を、在胎期間および性別で補正して算出した。観察感染症罹患率と推定感染症罹患率の差の解析から、参加施設によって院内感染症罹患率に大きなばらつきがあることが示された。3施設の院内感染症罹患率は、全集団の値を有意に下回った（P＜0.005)。これらの施設の臨床実践法の相違をさらに分析することにより、ネットワークにとって有益な実践法が明らかになると考えられる。

監訳者コメント：
新生児における院内感染症発生率は、生下時の在胎週数によっても著しく異なるので、その補正は不可欠である。成人とは異なり、非特異的バリアである皮膚も未形成なまま生まれてくることも多い。そういった意味では、この領域の研究も実践もこれからである。
