Outbreak of Serratia marcescens in a neonatal intensive care unit: contaminated unmedicated liquid soap and risk factors

本研究では、新生児集中治療室におけるセラチア・マルセセンス（Serratia marcescens）のアウトブレイクと、その調査および制御について報告する。3か月間で、乳児5例にS. marcescens単一株の保菌または感染が認められた。症例対照研究、培養調査、およびパルスフィールド・ゲル電気泳動解析から、S. marcescensの感染源としてソープディスペンサーのボトルが関与していることが示された（P＝0.032）。S. marcescens保菌・感染乳児は、中心静脈カテーテルまたは経皮的静脈カテーテル留置率が高く（P＝0.05）、気管内挿管日数が長かった（P＝0.05）。ソープディスペンサーは多くの病院で使用されており、院内感染源として認識されていない可能性がある。石けん使用時に空気の取り込みがない「エアレス」ディスペンサーを使用することで、この想定される感染源を抑制することができた。迅速な介入とアルコール製剤による手指消毒の厳格な遵守が、このアウトブレイク制御を成功に導く重要な因子であった。
