Development of a method to measure bacterial transfer from hands

医療従事者の手指からの細菌の伝播を調査する方法を開発した。この方法は、医療従事者と滅菌手袋を装着した被伝播者で手指の接触を標準化して、その後、グローブジュース法により手袋を装着していない医療従事者の手指および手袋を装着した被伝播者の手指から標本採取を行った。接触時間、摩擦の程度、および手指の乾燥状態にはばらつきがあった。人為的に大腸菌（Escherichia coli）で汚染した手指と自然汚染した手指からの細菌伝播について、乾燥した手指を中程度の摩擦で30秒間接触させて、その評価法の適用可能性を検討した。被伝播者の手指から回収された細菌数は、伝播者の手指に存在していた細菌数と比較するとごくわずかであり、大腸菌で0.15％、手指自然細菌叢で0.07％であった。手袋を装着していない手指から回収された大腸菌数は、手袋を装着した手指と比較して少数であったことから、自然状態の皮膚と接触したために、細菌の残存率が低下したことが示唆される。これらのデータは臨床的意義があり、また乾燥した手指との短時間の接触による細菌伝播はわずかであることを示していると考えられる。今回の方法は、多数の医療従事者が参加する臨床研究における、不適切な手指衛生の潜在的リスク因子の影響の調査、および接触伝播への手指衛生手順の影響の調査に適している。

やはり手指衛生につきる。なお、この論文の評価と直接に関係しないが、「手袋は手指衛生の代替ではない」という原則も再確認するべきである。
