医療従事者が衛生管理方法を厳格に適用することは、適切な清掃や消毒方法の実践とともに感染制御策の基礎となる。しかし、詳細な感染制御プログラムを導入している病院でも、院内感染は重大な問題となっている。したがって、生物医学研究者は、新たな感染制御策の有効性と安全性の評価を推進する必要がある。プロバイオティクス微生物は、環境表面上での院内感染病原体の増殖と拮抗する可能性があるとする予備的エビデンスが得られている。そこで著者らは、環境内のプロバイオティクス微生物は、感染制御のための安全かつ有効な介入法となり得るとの仮説を提唱する。著者らは、院内感染病原体の定着の減少を図る目的から、プロバイオティクスまたはその産生物質（バイオサーファクタント）をチューブやカテーテルなどの医療用器材に使用できると考える。これにより、院内感染の発生機序の重要な段階を阻害できる可能性がある。

環境表面においても黄色ブドウ球菌（Staphylococcus aureus）や腸球菌属（Enterococcus spp.）、大腸菌（E. coli）やクレブシエラ属（Klebsiella spp.）やアシネトバクター属（Acinetobacter spp.）、カンジダ属（Candida spp.）が生存することがあり、ここから感染伝播が拡大することがある。この対策として環境表面に乳酸桿菌（Lactobacillus spp.）などのヒトにとって有害性の少ないプロバイオティクス微生物を利用しようとする試みに関する総説である。腸内細菌叢ではなく病院細菌叢を整えようとする発想で興味深い。
