フィンランドのある3次ケア病院におけるノロウイルスGII.4-2006b亜型による長期間にわたったアウトブレイク★★

病院で発生したノロウイルスのアウトブレイクは制圧が困難である。アウトブレイク制圧にはコホーティングと接触隔離、表面消毒、手指衛生が重要な要素となる。ノロウイルス新種株GII.4.-2006bは多くの大陸に拡大しており、フィンランドでも2006年11月から2007年6月にかけて前例のない長期的流行を引き起こした。この論文ではフィンランドの大規模3次ケア病院におけるノロウイルスのアウトブレイクの臨床的および分子生物学的な特徴を報告する。2006年12月から2007年5月の間にヘルシンキ大学中央病院の本館504床で患者240例(18%)と医療従事者205例(19%)が発症した。流行曲線は1月、2月、4月に3回のピークを示したが、流行が発生した病棟はそれぞれで異っていた。アウトブレイク中に患者便検体502件のノロウイルスRNA検査を行ったところ、181件(36%)が陽性であった。陽性検体48件を対象とした分子生物学的解析から3種類の主要な GII.4.-2006b亜型がそれぞれ別の病棟へ時間を追って循環していたことが判明した。微生物学検査により確認された全症例のうち121例(67%)が院内感染伝播であり、9例(5%)が診断から30日以内に死亡した。分子的解析から3種類の主要なGII.4.-2006b亜型は複数の胃腸炎患者とともに院内に侵入していたこと、病棟への拡大は流行のピークと一致することが示唆された。一時的な病棟閉鎖などの積極的制御対策により、最終的にアウトブレイクは単一の病棟内にとどまった。市中におけるアウトブレイクの長期化が、病院内のアウトブレイクが長期化した原因であったと考えられる。

インフルエンザやノロウイルスなど、感染力の強い病原体が病院内に持ち込まれることはまれではなく、病院における感染管理にあっても市中における流行状況に敏感でなければならない。
