Severe consequences of healthcare-associated infections among residents of nursing homes: a cohort study

本研究の目的は、ノルウェーのナーシングホームにおける医療関連感染が、衰弱、病院への入院、死亡などに及ぼす影響を明らかにすることである。2004年10月から2005年3月に、ノルウェーの2つの主要都市のナーシングホーム6施設の入居者を追跡した。感染症がある各入居者に対して、感染症がない入居者から無作為に2名の対照者を選択した。症例と対照を1つのコホートとして30日間追跡し、両者の合併症発生率とリスク比（RR）を評価した。感染症発症率は1,000入居日数あたり5.2件であった。30日の追跡後に全般的な身体状態が悪化した割合は、感染症がある入居者の10.9％に対して、感染症がない群では4.8％であった（RR 2.3）。病院に入院したのは感染症がある入居者の13.0％に対して、感染症がない群では1.4％であり（RR 9.2）、追跡期間中にナーシングホームで死亡したのは感染症がある入居者の16.1％に対して、感染症がない群では2.4％であった（RR 6.6）。下気道感染症がある入居者は、合併症発生率および死亡率が高かった。結論として、医療関連感染は、ナーシングホームの入居者に対して衰弱、病院への入院、死亡などの深刻な影響をもたらす。

高齢化社会を迎え、医療を提供する施設も多様化している。医療関連感染の考え方もこうした多様性に適用していく必要がある。
