Hospital economic impact of an outbreak of Pseudomonas aeruginosa infections

集中治療室（ICU）の緑膿菌のアウトブレイクに関係した67例の患者を後向きに追跡し、症例患者の費用、入室期間、および生存率を非症例患者と比較した。病院で提供されたすべてのケア項目とサービスを詳細に特定し、算定する手法であるミクロ原価計算法（microcosting）を使用して、各患者の診断のための検査や手技、薬剤、およびICU入室に関連する帰属原価を明らかにした。院内緑膿菌感染症を発症した患者は17例であった。これらの患者に要した病院の補正費用は平均27,917ユーロで、非症例患者より66％高額であった（P＝0.002）。緑膿菌感染によるICU入室期間の延長日数は70日であった（P＝0.0001）。多重線形回帰分析では、緑膿菌感染は入院費用増加と入院期間延長の独立予測因子であった。これらの知見に基づき、当院のICUにおける緑膿菌感染による超過費用は、控えめな見積もりで312,936ユーロ（95％信頼区間305,676～320,196）であった。
