Antibiotic use: is appropriateness expensive?

抗菌薬は正しく使用されないことが多い。本研究では、3病院の600件の抗菌薬処方のうち、37％が不必要と判定された。また、適応内とされた抗菌薬療法のうち、45％は不適切と判定された。多変量解析から、適応内の治療は適応外の治療よりも高額であることが示された、これは適応外の治療では経口薬が多いためと考えられた。しかし、適応内の治療の中では、適切な治療と不適切な治療の費用に有意差は認められなかった。

監訳者コメント：
抗菌薬の適切な使用とコストを検討した興味深い論文である。そもそも抗菌薬投与が不要と判定された症例の割合や、抗菌薬投与が必要あると判断されても不適切な使用だと判定された症例の割合は、これまでも多く報告されており、妥当な数値であろう。不要と判定された治療が主に経口抗菌薬に偏っていることも想定内である。しかし、適切な使用であっても、不適切な使用より費用が多くないという事実は重要である。すなわち、コストを増大させることなく、感染症治療の知識によって患者の予後を改善できる可能性が示唆されている点が興味深い。
