Neurosurgically related nosocomial Acinetobacter baumannii meningitis: report of two cases and literature review

院内感染髄膜炎は脳神経外科処置の合併症としてはまれであるが、脳神経外科集中治療室（NSICU）ではグラム陰性桿菌による院内感染髄膜炎が発生することがある。アシネトバクター・バウマニー（Acinetobacter baumannii）は院内感染髄膜炎の原因菌としてはまれであり、NSICUでの髄膜炎アウトブレイクの原因菌となる頻度も非常に低い。脳脊髄液検体採取時の不適切な無菌操作が原因となった、NSICUにおけるA. baumannii髄膜炎の2症例を報告する。脳脊髄液検体を側脳室ドレナージの遠位ポートから無菌的に採取するという感染制御策を実施した後、新たな症例の発生はなかった。本報告では、脳神経外科の成人患者におけるアシネトバクター属による院内感染髄膜炎のレビューも実施している。

これは頻度としては少ないが、たった1症例でも髄液から分離された場合には、不適切なプラクティスが行われていないか監査しフィードバックすることが大切である。
