Perceptions of hand hygiene practices in China

手指衛生は、医療関連感染を予防するために最も重要な感染制御対策の1つと考えられている。しかし、推奨されている手指衛生の実施についての病院内の遵守率は依然として低い。手指衛生行動の改善方法に関するこれまでの文献は米国およびヨーロッパに焦点を当てたものであり、発展途上国からの研究は少ない。本研究の目的は、中国の病院で手指衛生の実施を改善させるうえで、共通の問題点と、想定される戦略を明らかにすることである。中国保健省が指定した8か所の病院の主要な病院職員および公衆衛生職員25名に対する詳細な聞き取り調査に基づき、定性的調査を実施した。病院職員は、手指衛生は効果的な感染制御に最も重要であると考え、正しい手指衛生の実施に関する適切な知識を有していた。このような積極的な姿勢と適切な知識にもかかわらず、正しい手指衛生の実施の遵守率を改善させるための重大な課題が判明した。それは、必要なリソースが不足していること、病院組織内での感染制御部門の権限が限られていること、および改善を動機づけるデータの監視とフィードバックが非効果的であることなどであった。中国で手指衛生の実施を改善させるための中心的課題は、経営上層部に対する感染制御部門の権限系統を明確にして、病院内の感染制御部門を重視し、優先権と影響力を付与することであることが、本研究から示唆される。感染制御部門を病院組織図の上位に配置し、サーベイランスのパラダイムを継続的な監視およびデータの効果的なフィードバックへと移行させることが、手指衛生の実施およびケアの質を改善するために不可欠である。
