Utility of spa typing for investigating the local epidemiology of MRSA on a UK intensive care ward

英国の集中治療室（ICU）では、メチシリン耐性黄色ブドウ球菌（MRSA）の地域的流行が頻繁にみられるが、ICU内のMRSAの局地的疫学に対する理解は十分でなく、MRSA感染を予防する最善の方法については異論がある。新規の分子タイピング法が、局地的なMRSA菌株の疫学研究に有用である可能性がある。英国のICU患者から採取したMRSA菌株に対して、ブドウ球菌プロテインA（spa）タイピングを実施した。spaタイピングはmulticolus sequence typing（MLST）より識別能が高いが、菌株の73％はspaタイプt032またはt018（英国のMRSA流行菌株であるEMRSA-15およびEMRSA-16に関連する）であった。患者の72％はMRSA感染に先立って保菌状態にあり、このうち88％は保菌株と起因菌株のMLST型およびspa型が同一であった。spaタイピングは、まれなspa型を有する19菌株については患者間のMRSA伝播の解明に役立ったが、EMRSA-15型およびEMRSA-16型のt032とt018の頻度が高かったため、多くの菌株ではspaタイピングは使用できなかった。意外なことに、まれなspa型の分離後28日以内に生じた45株の新規MRSA分離株のうち、交差汚染による可能性があるものは4株（9％）のみであった。これらの結果から、当ICUでは、少なくともまれなspa型の菌株の場合は、患者間のMRSA即時伝播はまれであることが示唆される。

遺伝子多型性検査にはPFGE法以外にも多くの方法が試みられている。効率よく分類するには、これらを組み合わせて解釈するしかないと考えられる。
