Hospital-acquired influenza: a synthesis using the Outbreak Reports and Intervention Studies of Nosocomial Infection (ORION) statement

院内感染インフルエンザのアウトブレイクは、ほぼすべての種類の病棟で発生しており、患者と病院に甚大な罹患、死亡、およびコストをもたらしている。あらゆる患者、医療従事者、および訪問者が病院内の感受性者にインフルエンザを伝播し得るため、感染源は不明であることが多い。院内感染インフルエンザのアウトブレイク調査により、感染源の特定、さらなる症例の発生予防、およびさらなるアウトブレイクに直面した際の疾患の制御に関する知識の増強に繋がるはずである。しかし、このようなアウトブレイクの検出と報告はおそらく十分には行われておらず、そのため伝播経路の追跡と正確な記述が困難となっている。さらに、文献中の情報の記載が標準化されていないことが、研究の比較と疾患動態の十分な理解の障害となっている。本研究では、院内感染インフルエンザのアウトブレイクに関する報告をORIONガイドラインに従って統合し、インフルエンザ症例の検出、患者と医療従事者間の伝播のエビデンス、および疾患の発生の指標に関する既存の知識について検討した。インフルエンザが病院内で蔓延していることは多くのエビデンスにより確認されているが、患者に対するリスク評価を目的とした発生率の算出のためには、臨床的およびウイルス学的判定法を改善し、アクティブ・サーベイランスと定量的研究を実施すべきである。

季節性インフルエンザの流行期間には医療従事者とて、罹患する。研修指定病院だと多くの職種の実習生や研修生も訪問しており、インフルエンザのような伝播力が強く、しかも健常者でも罹患する可能性のあるウイルス性感染症の場合、病院環境は伝播経路を特定するに容易な場所では全くない。地道な臨床実地疫学調査を行うしかない。
