Correlation between meticillin-resistant Staphylococcus aureus prevalence and infection control initiatives within southern and eastern Mediterranean hospitals

地中海地域は、多剤耐性院内病原菌の高度流行地と判定されている。こうした状況の背景にある潜在的要因への理解を深めるために、地中海地域抗菌薬耐性サーベイランスおよび制圧（Antibiotic Resistance Surveillance & Control in the Mediterranean Region；ARMed）プロジェクトに参加している27病院からのメチシリン耐性黄色ブドウ球菌（MRSA）に関する公表データと、同一施設からの感染制御および抗菌薬管理の様々な項目についての質問票への回答との相関を検討した。構造化質問票への回答スコアは、感染制御の態勢、手指衛生設備、および抗菌薬管理の実施については有病率の高い病院と低い病院との間に差は認められなかった。しかし、病床利用率と隔離施設のスコアでは差が認められ、過密状態が頻繁に発生し、特に複数の科に及ぶと報告した病院、および隔離ベッドの確保が日常的に困難である病院では、MRSAの割合が有意に高かった。この結果から、利用可能な病床数の不足に関連し、不適切な隔離設備により助長されるインフラの不備が、地中海沿岸南東部の病院におけるMRSAの高度流行を悪化させる可能性があることが示唆される。

設備の老朽化を理由に感染対策ができないとは限らない。手指衛生の遵守率をいかに高くし、維持させるかなど、検討すべき課題は山積しているはずだ。
