Personal protective equipment in an influenza pandemic: a UK simulation exercise

英国のパンデミックインフルエンザ感染制御指針を適用することが、個人用防護具（PPE）の使用、患者、および医療従事者に対してどのような影響を及ぼすかについての検証は、実務的および経済的な影響のいずれに関しても限られている。イングランド北西部の病院で実地演習を行い、これらの問題を評価した。24時間の演習中に、救急総合内科病棟の医療従事者全員がPPEを装着し、英国のパンデミックインフルエンザ感染制御指針に記されている手技を適用した。感染制御看護師のチームが、演習期間中のPPEの使用に対する態度などの医療従事者の行動と実践を観察・記録した。世界保健機関（WHO）による高レベルPPE（FFP3レスピレータ）の適切な使用に関する勧告は過剰であることが判明したが、グローブとサージカルマスクの使用は予想以上に多かった。予行演習を実施したにもかかわらず、多くの医療従事者はPPEの使用および感染制御対策の遵守を自信をもって行うことができなかった。医療従事者はPPEの装着に違和感を覚えており、基本的な業務にも通常以上の時間を要した。また、1日あたり12袋（570 L）の多量の医療廃棄物が余分に排出された。大量の高レベルPPEが必要であるという想定は、医療予算に重大な影響を及ぼすものであるが、今回の演習中のPPEの推定使用量は、この想定に対して意義を呈するものであった。継続的な感染制御教育プログラムが必要である。パンデミック中の医療は、パンデミックインフルエンザに対する感染制御指針を現行の業務に適用する一事例であるにとどまらない。病院は、ケアやサービスの提供方法の修正を検討する必要がある。

監訳者コメント：
新型インフルエンザについては、伝播経路が不明なためにウイルスの特性が明確になるまでは、過剰とも思える防護具装着が推奨される。しかし、平常時にこれら過剰なPPEを装着する機会が医療従事者全員に与えられている訳ではなく、技術水準としての防護性がすでに破綻している。
