How clean is clean? Proposed methods for hospital cleaning assessment

病院の感染率低下には、環境指標などの種々の因子が関与する。病院内の表面の衛生に関する微生物学的基準が提案されているが、環境サンプル採取の標準的な方法は論じられていない。本研究の目的は、清拭洗浄法による指標細菌検出およびディップスライドによる細菌数の定量的測定を用いた、重症管理室における清掃・消毒の有効性評価である。メチシリン感受性黄色ブドウ球菌（MSSA）とメチシリン耐性黄色ブドウ球菌（MRSA）の両方を対象として、臨床区域および非臨床区域で手の接触頻度が高い表面を微生物学的方法により調査した。好気性菌総数を測定するため、標的表面サブセットの定量的サンプル採取を行った。採取したサンプル130件中9件（6.9％）からMRSAが、15件（11.5％）からMSSAが分離された。非臨床区域から採取したサンプル81件中7件（8.6％）でMRSAの増殖が認められたのに対し、臨床区域で採取したサンプルでは49件中2件（4.1％）であった。好気性菌総数測定のスクリーニングを実施した116か所中9か所（7.7％）で、5 cfu/cm²を超える菌の増殖が認められた。総生菌数が多い表面は、ベッドのフレーム、電話器、コンピューターのキーボードなどであった。総生菌数とMRSA分離の結果に直接的な相関は認められなかった。しかし、両指標の併用により、清掃・消毒法の有効性を評価するためのより効果的な方法が得られるものと考えられる。特定の区域に必要とされる清掃の頻度を評価するために、あるいは使用されているプロトコールや資材を変更するために、これらの指標の評価・改善を目的としたさらなる研究が必要である。

監訳者コメント：
清掃の指標が感染率とリンクしない原因の1つに、保菌をしても必ずしも発病しないというドグマがあり、環境清掃がアウトブレイクのリスクを減らすことはわかっているが、どの程度減らすことができるのかについては、依然として不明である。よって、アウトカムの評価を感染率に依存している現在では、タイトルの問いに正確に答えることは不可能である。
