Epidemiology of community-acquired meticillin-resistant Staphylococcus aureus obtained from the UK West Midlands region

2005年1月から12月の間に、皮膚・軟部組織感染症のために地域の一般開業医（通称GP）を受診した外来患者から、メチシリン耐性黄色ブドウ球菌（MRSA）の分離株199株が検出された。調査対象とした地域は、英国イングランド中部のリッチフィールド、タムワース、バーントウッド、およびバーミンガム北部・東部の3つのプライマリ・ケアトラスト^※に属する57の診療所である。抗菌薬感受性試験、パルスフィールド・ゲル電気泳動法、Pantone-Valentine型ロイコシジン遺伝子の検出、およびSCCmecエレメントのマッピングの結果、分離株の95％は病院感染流行株EMRSA-15およびEMRSA-16と関連することが示された。分離株群の87％がSCCmec IVを、9％がSCCmec IIを有し、4％が新規のSCCmec IIIa^-mecI型と同定された。患者の自宅の郵便番号の地理的分布を調べたところ、SCCmec IIおよびSCCmec IVを有する分離株は広範囲に分布していることが判明した。しかし、SCCmec IIIa^-mecIを有する分離株の多くは、研究対象地域の北西部に居住する患者から検出され、局地的なクローン群である可能性が示された。本調査から示されたように、病院環境から周辺地域へのMRSAの伝播が生じた可能性があることから、標的患者のスクリーニングおよび臨床環境と市中環境の両方の除菌が必要であると考えられる。
