11年間に患者77例が集中治療室(ICU)への入室5日目以降にメチシリン耐性黄色ブドウ球菌(MRSA)菌血症を発症した。このうち10例にはMRSA陽性歴がなく、13例は入室時のスクリーニング結果が陽性であり、54例は血液培養が陽性となる以前にICUで実施した初回検査でMRSA陰性であった。この54例は、入室5日目以降にMRSAに感染した267例の20.2%[95%信頼区間(CI)15.6~25.0%]を占めた。MRSA菌血症患者77例の死亡率は57.1%(95%CI 46.0~68.2%)であった。この77例中19例には、血液中に他の病原体の増殖が認められた。MRSA菌血症のみの患者それぞれに対して、診断結果およびAcute Physiological and Chronic Health Evaluation(APACHE)IIの初回スコアに基づいて、MRSA菌血症がない対照患者5例ずつをマッチさせた。死亡率は菌血症患者[53.6%(95%CI 40.5~66.7%)]のほうが対照患者[31.8%(95%CI 26.3~37.3%)]より高く[相対リスク(RR)1.69(95%CI 1.25~2.26)、P<0.01]、死亡率の差の絶対値は21.8%(95%CI 8.0~40.1%)であった。この推定値を77例全例に適用すると、ICUで感染したMRSA菌血症によって17例(95%CI 6~31例)が死亡し、全入室患者の院内死亡率30.1%に対する寄与は0.3%(95%CI 0.1~0.6%)であることが示唆される。MRSA菌血症発生率はICU在室期間の延長に従って上昇し、25日以上ICUに在室した198例の死亡率37.9%に対するMRSA菌血症の寄与は3.1%(95%CI 1.1~5.7%)と推定された。これらのデータから、長期在室している患者の初期のMRSA保菌/感染を予防することの重要性が強調される。

監訳者コメント:
ICU長期在室がMRSA菌血症による死亡率を上昇させることが示された。ICUにおけるハイ・リスク群として認識する必要があるであろう。
