Hospital outbreak of Burkholderia stabilis bacteraemia related to contaminated chlorhexidine in haematological malignancy patients with indwelling catheters

バークホルデリア・セパシア（Burkholderia cepacia）complexは日和見病原体であり、院内アウトブレイクの原因となることがある。本稿では、血液悪性腫瘍患者にみられた汚染グルコン酸クロルヘキシジン溶液に関連するB. cepacia complexによる菌血症のアウトブレイクを報告する。B. cepacia complex分離株8株が、韓国の癌センター病院の同一病棟の入院患者から分離された。さらに、この病棟で使用している0.5％グルコン酸クロルヘキシジンからB. cepacia complex分離株3株が分離された。これらの分離株はバークホルデアリア・スタビリス（Burkholderia stabilis）と同定され、パルスフィールド・ゲル電気泳動法で同一のパターンを示した。B. stabilis菌血症患者はすべて静脈カテーテルを留置しており、これらのカテーテルはクロルヘキシジンで消毒処理されていた。汚染源が特定された後、消毒薬の監視培養による厳格な管理強化が実施された。それ以降、当病院ではB. stabilis感染は認められていない。

監訳者コメント：
消毒薬が汚染源になるという、あってはならない事態による感染アウトブレイクの事例である。国内でも過去にポビドンヨード製剤などの汚染事例があり、製品ならびに院内製剤においても常に留意する必要がある。
