クロストリジウム・ディフィシル(Clostridium difficile)感染の高リスク集団を判定することにより、C. difficile感染予防戦略の策定が可能になる。本研究の目的は、C. difficile感染高リスク者を予測する臨床的リスクインデックスの開発である。そこで、広域スペクトル抗菌薬投与を受けた患者コホートに基づき、開発コホートと検証コホートに分けて、C. difficile感染リスクインデックスを作成した。ロジスティック回帰式により、C. difficile感染の有意な予測因子を特定した。特定した危険因子を用いてC. difficile感染リスクのスコアリングアルゴリズムを作成し、4つのC. difficile感染リスクカテゴリーを設定した。受信者動作特性曲線下面積(aROC)で適合度を評価した。患者54,226例中392例がC. difficile感染陽性であった。C. difficile感染と有意に関連していた因子は、年齢50~80歳[オッズ比(OR)1.5、P<0.0116]、年齢>80歳(OR 2.5、P<0.0001)、血液透析(OR 1.5、P=0.0227)、非外科部門への入院(OR 2.2、P<0.0001)、および集中治療室入室期間が長いこと(OR 2.1、P<0.0001)であった。有意な関連があった因子の有無に基づく簡易リスクインデックスは、開発コホート(OR 3.57、P<0.001、aROC 0.733)および検証コホート(OR 3.31、P<0.001、aROC 0.712)のいずれにおいてもC. difficile感染リスクの増加と有意に関連していた。ORに基づいて作成したリスクインデックスの性能は、簡易リスクインデックスに及ばなかった。この簡便なリスクインデックスは、患者をC. difficile感染のリスクカテゴリーに層別化することが可能であり、予防戦略の策定に有用である。

本論文で提唱されている、C. difficile感染のリスクカテゴリー層別化は、患者管理上非常に有用である可能性がある。点数をつける方法は、年齢(50~80歳は1点、81歳以上は2点)、血液透析(1点)、内科系入院(1点)、ICUの入院日数(週単位)という非常にシンプルなもので、臨床応用も容易である。今後、多くの患者のコホートで検証されることが期待される。
