Investigation and management of an outbreak of multidrug-carbapenem-resistant Acinetobacter baumannii in Cambridge, UK

英国の多くの病院で、カルバペネム耐性の多剤耐性アシネトバクター・バウマニー（Acinetobacter baumannii）（MRAB-C）が流行している。ORION（Outbreak Reports and Intervention studies Of Nosocomial infection）の基準を使用して、2か所の集中治療室（ICU）で発生したMRAB-Cアウトブレイクを報告する。MRAB-C保菌・感染患者全員を対象とした。アウトブレイクの第1期に感染制御予防策の強化を導入した。第2期には成人脳疾患集中治療室（NCCU）の一部を封鎖し、患者の厳重な隔離、バリアナーシング、および週3回のスクリーニングを導入した。制御達成時にNCCUを再開し（第3期）、退院後の蒸気洗浄および月1回の排気・給気孔の清掃を行った。症例は19例で、その内訳はNCCU 16例、一般ICU 3例であった。患者の平均年齢は52歳で、6例が女性であった。全患者が人工呼吸下にあり、10例は脳室ドレナージを行っているか、頭蓋内圧モニタリング装置を装着していた。4例が菌血症を、他の1例は脳室炎を発症していた。9例には感染の臨床所見は認められず、4例は初回スクリーニング時に診断された。10例が治療を受け、8例が死亡した。環境検体から、NCCU全体が高度に汚染されていたことが示された。MRAB-Cは重症患者に感染・定着し、高い死亡率と関連する。今回のアウトブレイクは、早期の制御開始、患者の隔離、患者と環境のスクリーニング、および手指衛生、環境清掃、臨床的監視の強化により制御された。アウトブレイクの制御には多面的なアプローチが必須である。

監訳者コメント：
アシネトバクターは環境中に長期にわたり生存する厄介なグラム陰性菌である。今後日本でも集団発生の原因病原体として問題になってくると思われる。
