Investigation and control of a cluster of penicillin non-susceptible Streptococcus pneumoniae infections in a care home

高齢者介護施設の入居者2名が肺炎のために1か月間入院して、両名ともにペニシリン非感受性肺炎球菌菌血症で死亡した。この介護施設のすべての入居者および職員に対して、鼻咽頭スワブによるペニシリン非感受性肺炎球菌のスクリーニング検査を実施したところ、入居者1名と職員1名が無症候性保菌者であると判明した。保菌者にはリファンピシンを経口投与した。全4株はいずれも血清型14であり、複数部位配列タイピング（MLST）法により、スコットランドで未検出のST2652であることが示された。介護施設の入居者の調査から、肺炎球菌ワクチンの接種率が低いことが明らかになった（63％）。この結果は、一般集団の65歳以上の高齢者の接種率と同等であり、改善が必要である。

監訳者コメント：
ワクチンで予防できる疾患はワクチンで予防するべきである。成人では肺炎球菌多糖体23価ワクチン（PPV-23）、小児では肺炎球菌結合型7価ワクチン（PCV-7）が利用される。ワクチンによる予防は耐性菌に対しても有効であり、市中においてもペニシリン非感受性肺炎球菌が多いわが国では、肺炎球菌ワクチンの意義はさらに大きい。
