Obligatory occupational health check increases vaccination rates among medical students

2002年10月、エルランゲン・ニュルンベルク大学の臨床実習前のすべての医学生を対象として義務的職域健診を導入した。2005年から2007年の期間では約半数の医学生が臨床実習実施前に健診を受けた。この研究の目的は、臨床実習前の医学生に対する職域検診が、ワクチン接種率に効果を及ぼすかどうかを評価することである。臨床実習実施年の初頭に、連続した医学生242名（臨床実習前の職域健診実施121名および非実施121名）を調査した。医学教育の期間中のB型肝炎ワクチン接種率は女性で50％から96％、男性で58％から96％に上昇した。初期職域健診を受診しなかった医学生ではワクチン接種率が有意に低かった（女性85％、男性81％）。実習前検診が有意に有効であったのは、男性ではB型肝炎、破傷風、ジフテリア、ポリオ、風疹、および流行性耳下腺炎、女性でB型肝炎および風疹のワクチン接種であった。この研究では、特に男性のワクチン接種率の有意な増加を図ることができることを示している。臨床実習を開始した後の医学生でも、ワクチン接種率を向上させるためには個人単位の職域健診が必要である。ルーチンの職域健診は、ワクチン接種率の格差縮小に大きく寄与する可能性がある。

ワクチン接種は感染予防の基本であり、とくに医療従事者がB型肝炎ワクチンを未接種のまま医療現場に立つことは極めて危険である。ワクチン接種によりすべての医療従事者は護られるべきであり、医療機関による格差を許すべきではない。
