Surgical site infections in cardiac surgery after a hospital catastrophe

1999年11月2日、スペイン・サンタンデールにある900床の教育病院において、心臓血管外科に隣接する外部壁面の一部が崩落した。この研究の目的は、事故により院内感染および手術部位感染（SSI）のリスクが増加して、患者安全に影響を及ぼしたか否かを判定することである。直ちに院内感染防止策が強化された。1999年11月2日以前には連続217症例が手術を受け、それ以降は296例が手術を受けた。患者の疾患重症度、手術手技の複雑さ、入院期間は、いずれの研究期間も同等であった。この事故の前後における院内感染の発生率は、それぞれ28.1％、24.7％であった（P＝0.381）。呼吸器感染、尿路感染、および菌血症の発生率も同等であった。事故後の研究期間におけるSSI発生率は、統計学的に有意に低下した（14.8％対4.4％、P＝0.008）。外部壁面の崩落は、院内感染発生率の上昇と関連しなかったが、崩落後に実施した徹底的な感染制御策により手術部位感染の発生率は有意に低下した。

監訳者コメント：
病院における建設工事や改修工事により医療関連感染のリスクが増加するとされているが、この報告では5名の医療従事者が犠牲となるような大規模な建物の一部崩壊があったにも関わらず、外科医に対するSSI発生率のフィードバックを含めて、感染予防策の徹底させたことによりSSI減少を達成した。なお、事故後、心臓血管外科の入院病棟と手術室は別館に移動されていた。
