黄色ブドウ球菌性菌血症の成人における黄色ブドウ球菌性細菌尿症の合併は臨床転帰不良と関連する★

2000年1月から2003年12月までに、黄色ブドウ球菌性菌血症により当院を受診した患者における黄色ブドウ球菌性細菌尿の予後予測意義を判定するために、後向きコホート試験を実施した。合計378例が黄色ブドウ球菌の血液培養で1回以上陽性を示し、このうち221例に対して受診後24時間以内に尿培養を実施した。この群の206例の症例記録を調査のために入手することができた。これらの患者のうち、全例がメチシリン感受性黄色ブドウ球菌菌血症（MSSA菌血症）を呈し、35例（17％）の尿培養中に黄色ブドウ球菌が認められた。年齢、泌尿生殖器系の状態、および併存疾患で補正してロジスティック回帰分析を行ったところ、黄色ブドウ球菌性細菌尿症の合併は、ICU入室［リスク比（RR）2.5、95％信頼区間（CI）1.06～4.36、P＝0.04］および院内死亡（RR 2.18、95％CI 1.05～3.57、P＝0.04）の有意な危険因子であった。さらに、男性の脳血管疾患および両性での高年齢が、院内死亡および1年生存率と関連していることが明らかになった。黄色ブドウ球菌性菌血症患者の臨床転帰と黄色ブドウ球菌性細菌尿症との関連を調べるための前向き試験が必要である。
