被覆によりフォーリーカテーテルの閉塞をもたらす結晶性細菌バイオフィルム形成の観察★★

膀胱カテーテルを長期間留置している患者では、カテーテルが被覆により閉塞し、尿流が妨げられる合併症を来すことが多い。この問題は、ウレアーゼ産生菌、特にプロテウス・ミラビリス(Proteus mirabilis)感染と、これによるカテーテル上の結晶性バイオフィルム形成に起因する。本研究の目的は、P. mirabilisがどのように結晶性バイオフィルム形成を開始するかを解明することである。カテーテルを留置した膀胱の実験室モデルで、各種フォーリーカテーテル上のバイオフィルム形成の初期段階を観察した。オールシリコン、シリコンコーティングラテックス、ヒドロゲルコーティングラテックス、ヒドロゲル/銀コーティングラテックス、およびニトロフラゾン含浸シリコンカテーテルをP. mirabilisおよびアルカリ尿を含む膀胱モデルに挿入したところ、走査型電子顕微鏡写真により、各カテーテル表面は直ちに微結晶性の基層で覆われることが明らかになった。X線微量分析の結果、この物質の成分はリン酸カルシウムであった。次いで基層に細菌が定着し、18時間後には各カテーテルは高密度の結晶性P. mirabilisバイオフィルムで被覆された。今回の観察結果は、被覆抵抗性のカテーテルの開発に重要な影響を与えるものである。例えば銀カテーテルの場合は、細菌細胞は結晶性の基層のために基底の銀に接触することなく増殖し続けることができる。被覆予防のために抗菌薬をカテーテルに含浸させる場合は、抗菌薬を尿中に拡散させること、および結晶形成の引き金となるpH上昇を抑制することが重要である。

監訳者コメント:
Proteus mirabilisに限った実験室レベルでの分析結果ではあるが、尿道カテーテルの表面コーティング素材によらず、結晶性バイオフィルムが1日以内で完成してしまうという結果は驚くべきものである。著者らが最後に述べているように、今後の尿道カテーテルの開発方針に大きな影響を与える論文になるかもしれない。
