日本における再使用タオル由来のBacillus cereusによる院内感染

2000年から2005年の夏季に、バチルス・セレウス(Bacillus cereus)による院内感染の増加が認められた。2005年にはカテーテル使用に関連した血流感染が5例で認められた。原因株はそれぞれ異なり、新規のmultilocus sequence type(ST)であるST365、ST366、ST367、およびST368型であった。患者2例に由来する2つのST365株については、さらにパルスフィールド・ゲル電気泳動で判別を行った。再使用(洗浄後に乾燥または蒸気加熱)タオル(タオル1枚あたり>106 cfu)および病院のリネン室の洗濯機にB. cereusによる汚染が認められた。タオル由来のB. cereus株はST167、ST365、ST380、ST382に属し、これらの株の比率は患者由来株と同等または類似していた。B. cereus病院由来株はいずれも食中毒株(ST26、ST142、ST381)ではなかった。シプロフロキサシン耐性は病院由来株のみで認められた。食中毒株で通常みられる嘔吐毒素やサイトトキシンK遺伝子は、患者1例の分離株を除き病院由来株では検出されなかった。これらのデータから、遺伝子型、抗菌薬感受性、および毒性遺伝子のパターンが食中毒由来株とは全般的に異なる特定のB. cereus株が病院内を循環していること、また日本では特に夏季にタオルが重大な汚染源となることが示唆される。

B. cereusの菌株に関する詳細な分析であり、興味深い文献である。残念なのは、汚染された再使用タオルをどのようにすれば院内感染の増加を食い止めることができるのかが感染対策上関心の高い事項であるが、それについては本論文において言及されていない。
