内視鏡洗浄消毒器由来の細菌分離株の酸化剤耐性および交差耐性★

高水準消毒薬として二酸化塩素を使用していた洗浄消毒器から分離した細菌株を、過酢酸、二酸化塩素、および過酸化水素などの反応性の高い酸化殺生物剤に曝露し、分離株の感受性および交差耐性を調べた。各酸化殺生物剤に強い抵抗性を示した分離株2種[枯草菌(Bacillus subtilis)およびミクロコッカス・ルテウス(Micrococcus luteus)]に対する殺菌率を、標準的な懸濁試験により評価した。懸濁試験では、推奨「使用中」濃度では推奨曝露時間内に必要な殺菌効果が得られない場合があり、細菌数を105分の1に減少させるには60分間の曝露が必要な例がみられた。In vitroでは栄養型のグラム陽性菌分離株が酸化剤耐性を示すことがあり、類似化合物への交差耐性が生じる可能性がある。調査した2種類の細菌は反応性の高い殺生物剤に感受性を示すと考えられていることから、これらの分離株の生存を説明する耐性機序について、さらなる研究を行う必要がある。

監訳者コメント:
日本の医療現場において内視鏡の洗浄消毒の際に用いられる消毒薬は、過酢酸、グルタラール、フタラールである。しかし最近、電解酸性水などを用いる方法も徐々に普及しつつある。いずれの方法においても、本研究で述べられているような消毒滅菌耐性菌の問題は発生しうる。現場レベルで行えることは、洗浄消毒装置の生産者が定めた手順を遵守し、消毒薬に関しては濃度と消毒時間が守られているかどうかを確認することであり、定期的な耐性菌に関するサンプリングなどは必要ないと思われる。
