全米病院感染サーベイランス(NNIS)のリスクインデックスに基づく手術部位感染率の層別化は、簡便な手法と比較して、外科部門間の順位を比較するための良好な基準であるかどうかを検討するため、本研究を実施した。ドイツの全国的院内感染サーベイランスシステム(Krankenhaus Infektions Surveillance System;KISS)の234の外科部門が任意参加したサーベイランスデータを用いて、後向き解析を実施した。全体で、2001年1月から2006年6月の66カ月間に実施された12カテゴリーの外科手技による223,367件の手術の中から、4,275件の手術部位感染を調査対象とした。NNISの手法および米国疾病対策センター(CDC)の定義に従って、手術部位感染の能動的サーベイランスを実施した。各外科部門で手術手技ごとに以下の2種類の手術部位感染率、すなわち粗感染率、およびNNISリスクインデックスに基づくリスク補正後の標準化感染比(standardized infection ratio)を算出した。それぞれの感染率に基づいて外科部門の順位化を行った。両順位間の相関をSpearmanの相関係数(ρ)を用いて調べた。12カテゴリーの外科手技すべてにおいて、粗感染率と標準化感染比の間に強い相関が認められた(ρ>0.95)。比較の際に、標準化感染比の代用として理解が容易で記録が簡単な粗感染率を用いても、外科部門の順位が顕著に変動することはない。

監訳者コメント:
海外の基準をそのまま自国のサーベイランスに適用することにより、時には問題が生ずる。本論文のように評価基準の評価が必要であるが、この議論は我が国にもあてはまる。
