Prevention of hospital-acquired infections: review of non-pharmacological interventions

病院感染（院内感染）により罹患率、死亡率、および医療費が増加する。米合衆国に限っても、院内感染によって1年間で約170万件の感染が発生し、99,000人が死亡している。病院感染の伝播経路は表面（特に手指）、空気、水、経静脈経路、経口経路、手術など、多岐にわたる。手指や表面の適切な洗浄、良好な栄養状態、十分な看護師数、良好な人工呼吸器管理、コーティングされた尿道および中心静脈カテーテル、およびHEPAフィルタの使用などの介入は、いずれも院内感染率の有意な低下と関連している。複数の感染制御法および戦略を同時に実施すること（「バンドリング」）は、病院感染の罹患率と死亡率の低下を図る最良の方法となり得る。これらの感染制御戦略の多くは採算に合うだけではなく、院内感染に関連した医療費を削減することができる。多くの病院感染を予防するための様々な非薬理学的介入により、長期あるいは多剤による抗菌薬治療の必要性も減少する。抗菌薬使用の減少により、抗菌薬耐性菌のリスクが減少し、感染患者に対する抗菌薬の有効性が向上するはずである。

医療関連感染症を防止するためのアプローチに関する適切なレビューである。この総説にも取り上げられている “バンドル” とは、科学的に証明されている限定された数の標準的なケアをグループ化して実施するものであり、組み合わせて実践することにより大きな改善を達成することができると期待されている。治療よりも予防が重要であるとの認識が必要であり、最近の米国の流行では、感染管理担当者を “infection control professional” という呼称から “infection preventionist” に変更しようとの動きもある。
