感度、特異度、陽性反応的中率(PPV)、および陰性反応的中率(NPV)は、通常は診断検査関連用語である。これらの概念は診断法からサーベイランスシステムにまで拡大されているが、サーベイランスシステムは診断検査と同様ではない。サーベイランスシステムの感度と特異度を算出する場合には、PPV、NPV、および有病率のみが判明しているという状況があり得る。本研究の目的は、PPV、NPV、および有病率により感度と特異度を算出する2種類の方法の同等性を示すことである。感度および特異度の公式は第一原理に基づいて算出し、標準的な分割表を用いた計算値と比較した。この方法について、冠動脈バイパス術後の手術部位感染症例の標本調査を例として説明する。標本から算出した有病率は母集団の有病率の算出値であり、この値は、PPV、NPV、および有病率の関数である感度と特異度の公式の中で用いた場合にのみ、分割表からの算出と一致する。この原理の一般的証明は本稿の付録に記載した。PPV、NPV、および有病率のみが判明している場合には、サーベイランスシステムの感度と特異度は、2種類の方法で同等の算出値を得ることが可能である。
