Impact of preoperative screening for meticillin-resistant Staphylococcus aureus by real-time polymerase chain reaction in patients undergoing cardiac surgery

即日PCR検査を用いた術前スクリーニング導入後に心臓手術を実施した患者における、メチシリン耐性黄色ブドウ球菌（MRSA）による手術部位感染発生数の有意な減少を報告する。これは、英国南西部の1つの心臓外科部門で実施した観察的コホート研究である。著者らは、2004年10月から2006年9月までの期間に心臓手術のために入院した患者1,462例を研究対象とした。2005年10月から2006年9月までの期間に、患者765例に対してIDI MRSA PCR検査を用いて術前スクリーニングを実施した。保菌者であると判定された患者に対して、鼻腔内ムピロシン軟膏および局所トリクロサンによる治療を5日間実施し、周術期予防的抗菌薬としてフルクロキサシリンの代わりにテイコプラニンを単回投与した。この治療群の心臓手術後の手術部位感染発生率を、2004年10月から2005年9月までの期間にスクリーニングなしで手術を実施した患者697例と比較した。PCR法によるスクリーニング導入後は、全体の手術部位感染発生率は3.30％から2.22％に低下し、同時にMRSA感染率が有意に減少した（相対リスクの減少0.77、95％信頼区間0.056～0.95）。心臓手術時にPCR法によるスクリーニングと同時にMRSA抑制を図ることは、通常の臨床診療で実施すること可能であり、その後のMRSA手術部位感染の有意な減少と関連する。

監訳者コメント：
MRSAの保菌者を同定する積極的監視培養（active surveillance culture）は、MRSA感染のハイリスク患者において費用対効果に優れていることが大まかなコンセンサスになっている。本研究もそのような患者集団において、しかもPCR法による即日検査結果同定という手法を用いて、術後のMRSA感染症を激減させている。費用対効果についても述べられており、1例の術後MRSA感染症を減らすために113人の患者をスクリーニングすることが必要であり、その検査費用は日本円で約60万円であった。この費用をどう見るか、誰が負担するかによっても積極的監視培養の有用性については評価が分かれるところであろう。
