多剤耐性大腸菌による慢性創感染に対する簡便かつ効果的な治療法★★

抗菌薬耐性は、今日の治療上の重要な問題となっている。広域抗菌スペクトルを有する新規抗菌薬が登場しているにもかかわらず、感染部位から多剤耐性病原体を除去することは困難である。本研究では、多剤耐性大腸菌(MAREC)による慢性創感染症の治療として、クエン酸を単独の抗菌薬として用いる方法を開発した。MARECが培養陽性である慢性創感染34症例を対象とした。MARECに対するクエン酸の抗菌効果をin vitroの液体希釈法で評価した。3%クエン酸ジェルを各創傷に1日1回、創傷が完全に治癒するまで塗布した。最小発育阻止濃度が1,500~2,000 μg/mLのクエン酸により、全34分離株の発育が阻止された。3%クエン酸を創傷部位に7~42回、局所的に塗布した結果、多剤耐性大腸菌は感染部位から除去され、全34例の創傷が治癒した。この治療法は簡便で信頼性が高く、毒性がなく、効果的である。したがって、多剤耐性大腸菌による創感染症の治療には費用対効果の高いクエン酸の使用が推奨される。

監訳者コメント:
MARECのほかにも、例えばMDRPやESBL産生グラム陰性桿菌などでは同様の効果があるのか、今後の検討に期待したい。個人的には、大変好きなアプローチです。
