Long-term outcomes following infection with meticillin-resistant or meticillin-susceptible Staphylococcus aureus

黄色ブドウ球菌の病院および市中での抗菌薬耐性獲得が増加している。耐性および感受性黄色ブドウ球菌感染後の長期転帰の研究は行われていない。感染の転帰を評価するために、種々の部位からの培養でメチシリン耐性黄色ブドウ球菌（MRSA）またはメチシリン感受性黄色ブドウ球菌（MSSA）陽性を示した全患者を対象として、後向きのマッチドペア解析を実施した。入院期間および院内死亡率のほかに、全死亡率、再入院数、および感染後1年間の黄色ブドウ球菌培養結果などの長期転帰のデータを収集した。初回黄色ブドウ球菌感染から12カ月後に、患者の42％が死亡した。両群間に短期死亡率、入院期間、再入院数、および感染後1年間の黄色ブドウ球菌培養結果の差は認められなかった。しかし、MRSA感染はMSSA感染よりも退院後の高い死亡率と関連し、3カ月後32％対18％（P＝0.02）、6カ月後42％対22％（P＝0.002）、および12カ月後51％対32％（P＝0.005）であった。結論として、黄色ブドウ球菌感染は高い1年全死亡率と関連している。死亡の多くは退院後に発生している。感染後の1年死亡率は、MRSA感染患者のほうがMSSA感染患者よりも高い。
