Nosocominal hepatitis C in a thoracic surgery unit; retrospective findings generating a prospective study

C型肝炎ウイルス（HCV）感染を自覚していなかった胸部外科医から患者2例へのHCV伝播について報告する。非構造遺伝子5Bの部分配列決定および分子系統学的解析により、両患者のウイルスは外科医由来の遺伝子型1a株と密接な関連があることが判明した。さらに、胸部外科治療に関連した2例のC型肝炎症例が発見された。この2例のHCVの配列は相互に関連はあるが遺伝子型2bであり、感染源は明らかにならなかった。この問題の重要性を解明するために、胸部外科手術を予定している患者に試験への参加を呼びかけ、前向き研究を17カ月間にわたり実施した。手術前および手術から4カ月以上後に血液検体を採取した。術後検体で抗HCV抗体のスクリーニング検査を行い、陽性の場合は術前検体も分析を行った。術前の抗HCV抗体陽性が確認されたのは患者2例のみ（0.4％）で、観察期間中に抗HCV抗体陽性へのセロコンバージョンが認められた症例は、評価可能456例中1例もなかった。後向きに同定された症例が存在したものの、当部門のC型肝炎院内感染はまれであった。本研究は、C型肝炎に感染した従業員からの感染伝播のリスクを示すとともに、普遍的予防策の必要性を再認識させるものである。

監訳者コメント：
手術医や麻酔医がC型肝炎に感染し、他の患者にC型肝炎を伝播させた事例は他にもある。本報告は改めてその可能性を示したものであり、医療従事者が標準予防策を守ることにより患者から感染伝播を受けないこと、および患者に伝播させるリスクの高い手術医や麻酔医などは、定期的な健康診査によりC型肝炎のスクリーニングをすることが必要であると感じる。ぜひご一読いただきたい論文である。
