Mortality of patients with antibiotic-associated diarrhoea: the impact of Clostridium difficile

クロストリジウム・ディフィシル（Clostridium difficile）感染関連の死亡率に関する従来の研究は、相反する結果を示している。本研究の目的は、抗菌薬関連下痢症を呈する入院患者の短期および長期死亡率に対するC. difficile感染の影響を明らかにすることである。抗菌薬を投与されて下痢を発症し、糞便中のC. difficile TOX A/Bの酵素免疫アッセイを行った入院患者217例を対象として、前向き症例対照研究を実施した。Kaplan-Meier検定およびlog-rank検定を用いて単変量生存分析を行い、Cox回帰モデルで28日および長期死亡率の多変量解析を行った。組み入れ基準を満たした患者217例中52例（24％）がC. difficile TOX A/B陽性と判定された。全コホートの28日粗死亡率および長期粗死亡率は、それぞれ12.4％、56％であった。Cox回帰分析から、長期死亡率と関連する独立した統計学的に有意な変数は、低アルブミン血症、生活機能低下、および血清尿素レベル上昇のみであることが示された。C. difficile毒素陽性そのものは、短期または長期死亡率の増加と関連はなかった。結論として、低アルブミン血症、腎不全、および生活機能低下は抗菌薬関連下痢症による死亡を予測したが、C. difficileの関与自体はこれらの患者の死亡リスクをさらに増加させることはなかった。
