Dissemination of Bacillus cereus in a paediatric intensive care unit traced to insufficient disinfection of reusable ventilator air-flow sensors

2006年11月の2週間にわたり、小児集中治療室（PICU）で人工呼吸器を装着した患児6例の呼吸器検体からバンコマイシン耐性バチルス・セレウス（Bacillus cereus）が分離された。想定される共通感染源および伝播範囲を調べるために、人工呼吸器に関連するすべての処置をモニターし、監視培養を行った。70％アルコールによるPICUでの消毒の前後に、再利用可能な気流センサーからB. cereusが分離された。人工呼吸器を装着したその他の患児3例の呼吸器検体、およびPICUの天井に設置された2個の換気格子、およびアルコール溶液自体からもB. cereusが分離された。増幅断片長多型（AFLP）型別により、PICUの患児6例に由来するB. cereus株と、気流センサーおよびアルコール溶液から回収した分離株との密接な関連が認められた。換気格子から分離された株は、アウトブレイク株とはAFLPパターンが異なっていた。すべての再利用可能な気流誘導用の部品に対する高圧蒸気滅菌や、高圧蒸気滅菌不可の部品を使い捨て用にするなどの介入策により、アウトブレイクは速やかに終息した。B. cereus感染症は、特に集中治療室の患者に重大な疾患を引き起こす可能性がある。人工呼吸器の気流誘導用の再利用可能なすべての部品の消毒には細心の注意が必要であり、芽胞を殺滅するために効果的な高圧蒸気滅菌を行う必要がある。

日本でもB. cereusによる感染症の伝播が報告されているが、不十分な再利用可能物品の消毒による伝播は比較的珍しくない。
