Hospital-associated infections during and after care in a paediatric infectious disease ward

ウイルスは小児科の病院関連感染症の重要な原因となっている。著者らは、小児感染症病棟の入院中および退院後のウイルスによる病院関連感染症の頻度について評価を行った。データの収集は、新たな感染症の症状を親が報告する経過観察質問票を用いて、2年にわたり実施した。入院後72時間以降または退院後72時間以内に発生した感染症を、病院関連感染症とした。患者の平均年齢は3.0歳、平均入院期間は3.0日であった。入院中に病院関連感染症を発症した患者は1,927例中21例［1.1％、95％信頼区間（CI）0.7～1.7］で、全例が下痢であった。合計1,175件（61％）の質問票を回収した。全体で、退院後72時間以内に新たな症状が発現した小児は86例（7.3％、95％CI 5.9～9.0）で、最も頻度が高いものは下痢（49％）であった。年長児は病院関連感染症が少なかった［オッズ比（OR；1歳ごと）0.92、95％CI 0.85～0.99、P＝0.02］。呼吸器感染症で入院している患者では、相部屋であることが病院関連感染症のリスクを高めた（OR 2.3、95％CI 1.1～4.8、P＝0.03）。本病棟では、アルコール手指消毒薬のハンドジェルを積極的に使用し、1人部屋が一般的であるが、患者の8％が病院関連感染症を発症した。病院関連感染症の80％は家庭で発症したことから、退院後の観察の重要性が示された。

筆者の施設は小児感染症病棟の多くが個室であり、アルコール系手指消毒薬を使用しているが、それでも退院後に多くの病院関連感染が発生している。小児感染症病棟での病院感染制御の困難さを感じさせる論文である。
