Incidence of healthcare-associated infections in high-risk neonates: results from the German surveillance system for very-low-birthweight infants

出生体重が1,500 g未満の乳児（極低出生体重児）では、医療関連感染のリスクが高い。この研究では、2000年から2005年にドイツの52の新生児科で収集したNEO-KISSサーベイランスシステムからのサーベイランスデータを示す。乳児を2種類の出産体重分類（1,000 g未満および1,000 gから1,499 g）で層別化して、病院内血流感染、病院肺炎、および壊死性腸炎の発生率を算出した。示したデータは、極低出生体重児8,677例、延べ339,972患者･日を対象とする。血流感染発生率は、1,000患者･日あたり6.5件（出生体重分類別では8.5および4.0件）であった。中心静脈カテーテル（CVC）関連血流感染発生率は1,000 CVC･日あたり11.1件、末梢静脈カテーテル（PVC）関連血流感染発生率は1,000 PVC･日あたり7.8件であった。肺炎発生率は1,000患者･日あたり0.9件（出生体重分類別では1.3および0.4件）であった。挿管患者における肺炎発生率は1,000人工呼吸器･日あたり2.7件であったが、経鼻的持続陽圧呼吸療法（CPAP）を実施している患者の肺炎発生率は1,000 CPAP･日あたり1.0件であった。壊死性腸炎の発生率は1,000患者･日あたり0.9件（出生体重分類別では1.1および0.6件）であった。医療関連感染は極低出生体重児で頻度が高く、新生児科ごとのばらつきが大きかった。これらの乳児の感染を減少させるための予防戦略を優先的に実施すべきである。

監訳者コメント：
極低出生体重児の医療関連感染対策は極めて重要な課題である。新生児医療の発展に対応することが出来る感染対策でありたいと考えているが、一方では、感染対策の充実こそが新生児医療の発展に繋がる。
