Virulence determinants in vancomycin-resistant Enterococcus faecium vanB: clonal distribution, prevalence and significance of esp and hyl in Australian patients with haematological disorders

欧州での研究によれば、esp遺伝子やその他の病原性決定因子が、バンコマイシン耐性エンテロコッカス・フェシウム（Enterococcus faecium）（VREfm）感染症に関与していることが示唆されている。本研究の目的は、vanB型VREfm分離株の疾患スペクトルと推定的な病原性決定因子との関連を調べることである。オーストラリアの血液腫瘍患者から採取したVREfm分離株において、病原性遺伝子と考えられる遺伝子（asa1、gel E、cylA、esp、hyl）を、マルチプレックスPCR法により増幅した。クロナリティについては、パルスフィールド・ゲル電気泳動（PFGE）および自動化リボタイピング法により評価した。菌の病原性の臨床的指標として、感染症、集中治療室（ICU）への入室の必要性、30日全死亡率を用いた。vanB型VREfm分離株41株（患者41例；感染14例、保菌のみ27例）を解析対象とした。これらの分離株のうち35株の遺伝子型別をPFGEで行ったところ、31株に3種類の代表的クラスターが認められた。esp遺伝子は、保菌株27株中22株（81.5％）および感染症に関連した分離株14株中11株（78.6％）で同定された。hyl遺伝子陽性は1株であり、asa1、gel E、またはcylAを保有する分離株はなかった。VREfm感染症は、宿主因子（基礎疾患として急性骨髄性白血病の診断、60歳以下）と独立して関連していたが、esp遺伝子の存在との関連は認められなかった。ICUへの入室は、esp遺伝子の存在と負の関連が認められた（OR 0.05、95％CI 0.01～0.61、P＝0.02）。30日死亡率と、宿主因子またはesp遺伝子の存在との間には関連はなかった。本研究の免疫低下集団から採取した多クローン性のバンコマイシン耐性腸球菌（VRE）分離株では、欧米の報告と比較してesp遺伝子の保有率が高く、hyl遺伝子の保有率が低かった。
