Reassessment of the risk of healthcare-acquired infection during rigid laryngoscopy

硬性喉頭鏡の不適切な再処理が、院内アウトブレイク、およびこれに関連する疾患と死亡をもたらしている。昨年、新生児集中治療室で緑膿菌のアウトブレイクが複数の感染および保菌の原因となり、少なくとも2例の乳児がこれにより死亡した。このアウトブレイクの調査によって、感染源は汚染された硬性喉頭鏡であることが判明し、硬性喉頭鏡の不適切な再処理が依然として現在の公衆衛生上の問題であることが示された。本稿では、硬性喉頭鏡検査中の医療関連感染リスクを再検討・再評価するとともに、硬性喉頭鏡のブレードおよび持ち手の再処理に最低限必要とされる条件を明らかにする。微生物伝播をもたらす潜在的ないくつかの危険因子について、見解が一致していない既存の硬性喉頭鏡再処理ガイドラインなども対象として、特定し、検討する。硬性咽頭鏡の低～中水準消毒を推奨しているガイドラインへの懸念を述べる。また、硬性咽頭鏡をカバーして汚染リスクを最小化する滅菌ディスポーザブルシースの使用についても考察する。処置中にシースを使用しているかどうかにかかわらず、微生物の伝播を予防するためには、器具の徹底した洗浄と、その後の高水準消毒および乾燥が推奨される。
