入院患者の基質特異性拡張型β-ラクタマーゼ産生大腸菌感染:死亡率に影響する因子★

大腸菌感染患者の死亡の危険因子を判定するために、後向きマッチドコホート研究を実施した。1996年1月から2003年12月に、基質特異性拡張型β-ラクタマーゼ(ESBL)産生大腸菌感染入院患者100例を、ESBL産生大腸菌感染がない患者と比較した。これらの対照患者は、感染部位が同一であること、および入院日がごく近いことを基準として選択した。両群の比較により、ESBL産生大腸菌感染患者では不適切な経験的抗菌薬投与が多く(44%対15%、P<0.01)、早期死亡率(16%対6%、P=0.02)および全死亡率(25%対11%、P=0.01)もESBL産生大腸菌感染患者で有意に高いことが示された。多変量モデルから、大腸菌感染の早期死亡に影響する唯一の独立危険因子は、尿路の病巣であることが判明した[オッズ比(OR)0.1、95%信頼区間(CI)0.03~0.7、P=0.01]。オキシイミノβ-ラクタム系薬を初期に投与したESBL産生大腸菌尿路感染患者12例は、全例が生存していた。尿路以外の大腸菌感染患者130例のコホートを対象として引き続き行った、早期死亡に影響する因子の解析からは、不適切な経験的抗菌薬投与が尿路以外の大腸菌感染のみの独立危険因子であることが示された(補正OR 3.0、95%CI 1.0~8.6、P=0.03)。本研究から、ESBL産生大腸菌感染入院患者では不適切な経験的抗菌薬投与が多く、非ESBL産生菌株感染患者よりも死亡率が高いことが示された。感染部位は死亡に強く影響する。不適切な経験的抗菌薬投与は尿路以外の感染患者の場合のみ、独立して高い死亡率と関連する。

監訳者コメント:
適切かつ迅速な診断と、経験的抗菌薬の適切な選択が、抗菌薬耐性の大腸菌感染症を防止するということになる。診断はともかく、検体培養の結果を得るまでの経験的抗菌薬の選択については感染症科医師へのコンサルトやガイドラインが必要であろう。日本の状況はその点において、近年急速に進歩がみられている。
