病院感染による財政的および人的コストは、多くの医療システムで認識が広まりつつある。本研究の目的は、2003年および2004年のベルギーの3つの総合病院における病院感染菌血症による超過支出を算出することである。病院感染菌血症患者を、All Patient Refined Diagnosis Related Group(APR-DRG;全患者を対象とした精密化した診断群分類)が同一であり病院感染菌血症を発症していない患者と比較した。患者レベルのコストをブリュッセル自由大学(Universite Libre de Bruxelles)が開発した病院費用計算システムで推計し、これらの3病院のDRGベースの医療報酬(funding)と比較した。病院感染菌血症の発生率は、3病院のうち2病院で全国発生率と一致していたが、1病院では全国発生率よりもかなり高かった。疾患の重症度および死亡率は病院感染菌血症群のほうが高かった。病院感染菌血症は、在院日数の30日の増加、および集中治療室在室日数の6.1日の増加と関連していた。DRGが同一である非感染患者と比較して、病院感染菌血症患者の治療には16,709ユーロの追加費用を要していた。現在の医療報酬率で、病院は、非感染患者からは平均446ユーロの利益を得ているが、病院感染菌血症患者については平均2,431ユーロの損失であった。本研究の所見から、APR-DRGシステムはこのような合併症に対して重症度を調整し、医療費を拠出する仕組みでありながらも、病院は病院感染菌血症の発生率を減少させることによって財政上の利益を得られることが示唆される。実績に応じた支払い(pay for performance;P4P)やその他の医療報酬算定法に対する関心が国際的に高まると、このような財政上のインセンティブにいっそう拍車がかかるであろう。

監訳者コメント:
包括医療を行っている場合、医療関連感染による在院日数の伸びが経営収支を大きく左右する。米国のSENIC研究はこの分野の研究として有名であるが、自国のデータを検証することは極めて重要であろう。
