Intensive care unit environmental cleaning: an evaluation in sixteen hospitals using a novel assessment tool

隔離予防策の励行および手指消毒用品の使用の普及にもかかわらず、医療関連病原体の伝播は依然として重大な問題である。集中治療室（ICU）環境の微生物汚染が、患者の保菌および感染をもたらすことが、最近の研究で確認されている。病原微生物の蔓延を最小限にするために環境消毒薬が使用されるが、不十分な清掃はこれらの効果を制限する可能性がある。患者周辺の表面清掃の程度を評価するために、透明かつ清掃が容易で、環境中の安定性が高く、紫外線で蛍光を発する溶液を開発した。患者退院時の清掃後、患者の接触頻度が高いとして標準化されているICU内の部位を、この溶液でマーキングした。2例以上の患者が使用した病室を使用し、2回以上の最終清掃の完了後に、これらの部位の評価を行った。患者197例の区域内の2,320の部位を評価したところ、評価対象とした16のICUでは、患者の退院後、標準化されている部位の清掃の程度は57.1％であることが明らかになった。便座、シンク、トレイテーブルの清掃の程度は高かったが（＞80％）、便器洗浄器、トイレ区域の取手、ドアノブ、および照明用スイッチなどの部位は一貫して清掃の程度が低く（30％以下）、院内感染病原体による汚染リスクが高いことが示された
