新生児は医療関連感染症に最も罹患しやすい患者群であり、複数の内因性および外因性のリスクを有している。新生児の血流感染症(BSI)の発生率を解釈するためには、ケースミックスの層別化が必要である。ある単一の新生児治療室に入室した連続1,367例を34カ月にわたって評価した。ポアソン回帰分析により、4種類の内因性リスクと7種類の外因性リスクを単独および組み合わせで評価した。単変量解析では、対象とした11種類の危険因子中の9種類にBSIとの有意な関連が認められた。独立した有意の危険因子としては、中心ラインまたは末梢ラインによる非経口栄養法[発生率比(IRR)14.2、95%信頼区間(CI)8.8~22.9、P<0.001]および妊娠26週未満(IRR 2.5、95%CI 1.7~3.8、P<0.001)のみとなった。この両方の危険因子を有する新生児の1,000患者・日当たりのBSI発生率は、いずれの危険因子も有しない新生児の40倍以上であった。他の環境においても妥当性が検証されるならば、非経口栄養法と妊娠26週未満という2つの明確な危険因子を用いて新生児のBSI発生率の層別化を行うことは、病院内および病院間の比較を意味あるものとする容易な方法となる。

監訳者コメント:
サーベイランスは継続的な医療質保証のための手段であるのが第一義であるが、他施設との比較やベンチマークとしての評価に使用するためにはリスクによる層別化が必要である。数字が独り歩きしてしまうと解釈を誤る。
