Endoscope decontamination incidents in England 2003-2004

2004年5月の内視鏡汚染除去事故の報告後に、内視鏡対策委員会（Endoscope Task Force）が設立された。対策委員会は、2003年から2004年の英国の内視鏡汚染除去事故を調査し、再発防止のための勧告を行った。19の国民健康サービス（NHS）トラストから21件の事故の報告があり、このうち18件が対策委員会の事故の定義に合致した。補助内視鏡チャンネルの汚染除去不成功が8件の事故にかかわっており、自動内視鏡洗浄装置の問題が7件の事故で浮き彫りになり、残る3件では英国消化器病学会（British Society of Gastroenterology）のガイドラインが推奨していない消毒方法がかかわっていた。血液媒介ウイルス伝播のリスク評価後に、対策委員会は遡及調査は不要との勧告を行った。事故の特性から、内視鏡汚染除去に対する役割と責任の明確化、職員の訓練、および内視鏡と洗浄装置との不適合性に関連する問題があることが示唆された。これに続いて、英国医薬品庁（Medicines and Healthcare Products Regulatory Agency）は、内視鏡検査を実施しているすべてのNHSトラストに対して勧告を行った。

監訳者コメント：
英国の内視鏡汚染除去事故の現状が把握できる。国としてのこのような対応は、個々の医療従事者レベルでは参考にならないかもしれないが、日本も国レベルで見習うべき方法なのかもしれない。
