Human immunodeficiency virus postexposure prophylaxis for occupational exposure in a medical school hospital in Thailand

本研究は、タイ、バンコクのKing Chulalongkorn Memorial Hospital（KCMH）の医療従事者での、ヒト免疫不全ウイルス（HIV）に対する職業曝露およびその後の曝露後予防治療に関する後向き再調査である。2002年1月から2004年12月まで、事故報告、病院の感染症部門および救急科からデータを収集した。医療従事者306名の職業曝露の315件のエピソードの報告があった。最も曝露が多い医療従事者は看護師（34.0％）で、曝露の種類としては経皮的損傷（91.4％）が最も多かった。検査した感染源患者の3分の1は、HIVに感染していた。315件の曝露中200件（63.5％）は曝露後に曝露後予防治療を開始し、95％を超える症例は24時間以内に開始していた。最も多く処方された曝露後予防治療レジメンはジドブジン＋ラミブジン＋ネルフィナビルであった。曝露後予防治療を受けた医療従事者の56％が4週のコースを完了した。残りは副作用により、または感染源患者の陰性判明後、リスク再評価の結果の通知後、または自らの判断で、早期に曝露後予防治療を中止した。曝露した医療従事者の中に、研究期間中にHIVに感染した者はいなかった。医療従事者間での適切なカウンセリングおよび慎重なリスク評価が、有効なHIV曝露後予防治療を達成するうえで重要である。

監訳者コメント：
3年間でのHIV職業曝露が315件は多いと感じるが、曝露後予防治療の体制が整っていること、および職員がすぐに曝露後予防治療を求めて職業曝露担当部署などに相談している様子がうかがえる。首都の病院とはいえ、発展途上国における体制の充実度合いは参考になる。
