Isolation of Legionella species/serogroups from water cooling systems compared with potable water systems in Spanish healthcare facilities

2005年から2006年にスペインの入院医療施設44カ所で、病院給水システムのレジオネラ属菌のサーベイランスを実施した。冷却システム（冷却塔）から470および飲料水システムから1,871、合計2,341のサンプルを採取した。飲料水システムのサンプルの内訳は、冷水タンクから211、給湯タンクから260で中央貯水槽からの合計は471、またシャワーから136、フィルターがない蛇口から1,172、フィルター付きの蛇口から92で給水端末からの合計は1,400であった。温度と塩素濃度を測定し、レジオネラ属菌の有無を検査した。全サンプル中373（15.9％）にレジオネラ属菌が認められた。冷却塔（23.8％）からの分離率が冷水タンクおよび給湯タンク（約4.7％）と比較して有意に高かったが、これは血清群1陽性サンプルが有意に多いことによる（19.4％対0.9～3.5％）。飲料水システムでは、中央貯水槽とシャワーの間に差は認められなかったが、中央貯水槽とフィルターがない蛇口との間では、血清群1以外のレジオネラ・ニューモフィラ（Legionella pneumophila）の相違により分離率に有意差が認められた（4.7％対19.6％）。フィルターにより血清群1以外の分離率は有意に低下した（11％対0％）。季節差がいくつか認められ、冷却システムのL. pneumophila血清群1および飲料水システムのL. pneumophila血清群2～14は、夏季の分離率が高かった。回帰モデルでは、冷却システムでは高温が定着と関連していたのに対し、飲料水では低い塩素濃度が定着と関連していた。
