Outbreak of multidrug-resistant Acinetobacter baumannii in a Belgian university hospital after transfer of patients from Greece

多剤耐性アシネトバクター・バウマニー（Acinetobacter baumannii）は重要な院内感染病原体になりつつある。これらの菌は、病院環境で長期生存することが可能である。本研究の目的は、ベルギーの大学病院でギリシャから外傷患者2例の転院後に発生した多剤耐性A. baumanniiによるアウトブレイクの経過および制御について記述することである。両患者の創傷部位に多剤耐性A. baumanniiが定着していた。早期に接触予防策を実施したにもかかわらず、2004年9月から2005年7月までの11カ月間に、患者28例からカルバペネム非感受性A. baumannii（カルバペネム加水分解オキサシリナーゼOXA-58産生）が分離された。多剤耐性A. baumanniiは患者26例のルーチンの臨床診断サンプル、および患者2例のスクリーニングサンプルから検出された。患者20例（71.4％）は、集中治療室在室中に保菌または感染した。24例（85.7％）の呼吸器サンプルは多剤耐性A. baumannii陽性であった。気道関連の全処置を精査したが、共通の伝播経路は特定できなかった。アウトブレイク制御には、保菌患者および感染患者の接触隔離策、職員の作業の監視、無症候患者のスクリーニング、隔離病室の使用、および環境消毒の強化など複数の介入を必要とした。使い捨て人工呼吸器回路の導入を検討したが、実施前にアウトブレイクは制御された。

監訳者コメント：
院内感染の起因菌としての多剤耐性菌として、MRSA、MDRP、VREについで注目を集めつつあるのが多剤耐性A. baumaniiである。日本でも今後集団発生の起因菌として問題になるポテンシャルを秘めており、要注意の病原体である。本事例はその意味で大いに参考になると思う。
